General Questions

Can I fill out registration forms prior to my visit?
Yes. Go to Patient Forms and print, complete and bring with you to your visit. Be sure to bring in your insurance card and a photo id so we can make a copy.

How do I know if I need a referral?
Click on our Insurance section of the website, on our participation list we have provided where an authorization/referral is needed. If you are still unsure whether or not your insurance policy requires a referral check with your insurance company.

Will I be seen without a referral?
No. You will need to reschedule your appointment.

How long should I expect to be there during a scheduled visit?
We do our very best to see patients in a timely manner. We apologize in advance should there be a lengthy wait. We try to give the best attention to our patients and their concerns and needs. We will give you the same attention we give every patient before and after you.

What if I have a reaction to a prescription?
Stop using the prescribed drug immediately. Contact us if the reaction is not an emergency, dial 9-1-1 if you are in need of urgent attention.

What should I do/not do if I have just had surgery?
